I confirm that I have read, understood and comply with the terms and conditions of the Kent County Council Household Waste Recycling Centre voucher scheme. Failure to do so will result in vehicle access being suspended and/or denied. I understand that providing false information may constitute an offence under The Fraud Act 2006 and breaches of the above may leave me liable to prosecution.

My household's sole vehicle is the above vehicle, which is the subject of this application, and none of the other members of my household either own or have use of an alternative vehicle registered to the household above.

I confirm that all the waste brought to a site to which I (or the additional driver on my behalf) have gained access to using a voucher will be my household waste from the address stated above only.

I understand that any unauthorised deposit of business or commercial waste at this site is an offence under the Fraud Act 2006 and section 34 of the Environmental Protection Act 1990.
